Случай двустороннего нетравматического заднего переломовывиха головок плечевых костей

   Задний вывих плечевого сустава представляет собой редкую патологию, доля которой составляет от 1,5 % до 5 % всех случаев вывихов, наблюдаемых в данной области. Сочетание заднего вывиха с переломом плечевой кости является ещё более необычным клиническим случаем.

   Клинический случай: Пациент Р., 54 лет, обратился с жалобами на боль и ограничение движений в обоих плечевых суставах, возникшие около 3 недель назад внезапно вслед за судорожным припадком, случившимся после эпизода употребления алкоголя. При КТ был выявлен двухсторонний задний переломовывих обеих плечевых костей и компрессионный перелом Th5 позвонка 1 ст. Двусторонние задние переломовывихи плеча — крайне редкая патология, чаще всего связанная с эпилептическими припадками или сильными травмами. Основные механизмы повреждения включают прямой удар и чрезмерные мышечные усилия. Диагностика таких повреждений нередко затягивается, однако внимательный сбор анамнеза позволяет заподозрить данную патологию. Рентгенография является ключевым методом диагностики с патогномоничными признаками, но для лучшей визуализации предпочтительна компьютерная томография. Лечение включает как консервативные, так и хирургические методы — в зависимости от степени повреждения и состояния пациента. Таким образом, несмотря на развитие методов визуализации в большинстве случаев диагностика заднего переломовывиха затягивается более чем на 6–12 месяцев, что может оказать существенное влияние на тактику ведения пациента. Тщательный сбор анамнеза и своевременно проведённая лучевая визуализация позволяют установить правильный диагноз, что, в свою очередь, помогает снизить риск возникновения рецидивирующей нестабильности.
