Проведён аналитический обзор литературы по современным подходам к проблеме дисфункции (гипофункции) мочевого пузыря в послеродовом периоде, наиболее частым клиническим проявлением которого является послеродовая задержка мочи. Многофакторный патогенез данного осложнения включает нейромышечные повреждения, механическую обструкцию, влияние анестезии и болевой синдром. Современные исследования позволяют чётко определить группы риска (инструментальные роды, эпидуральная анестезия, длительные роды, крупный плод), что даёт возможность врачам проводить целенаправленный скрининг. Внедрение протоколов раннего выявления (УЗИ-сканирование, измерение объёма мочи) и унифицированных подходов к лечению позволяют минимизировать риски и обеспечить благоприятный долгосрочный прогноз для пациенток с данным осложнением. Данные литературы проиллюстрированы собственным наблюдением успешного ведения пациентки с гипофункцией мочевого пузыря в послеродовом периоде.</p></abstract><trans-abstract xml:lang="en"><p>The article presents an analysis of a clinical case of neurogenic urinary disorder in the postpartum period after spinal analgesia. An analytical review of the literature on modern approaches to the problem of bladder dysfunction (hypofunction) in the postpartum period, the most common clinical manifestation of which is postpartum urinary retention, has been conducted. The multifactorial pathogenesis of this complication includes neuromuscular damage, mechanical obstruction, the effect of anesthesia, and pain syndrome. Modern research makes it possible to clearly identify risk groups (instrumental labour, epidural anesthesia, prolonged labour, large fetus), which allows doctors to conduct targeted screening. The introduction of early detection protocols (ultrasound scanning, urine volume measurement) and unified treatment approaches can minimize risks and ensure a favourable long-term prognosis for patients with this complication.
