Описаны изменения микроциркуляции, нарушения аксонального транспорта при компрессии спинномозгового корешка (СМК) грыжей межпозвонкового диска (МПД), отражена динамика возможной регрессии грыж МПД. Описаны патофизиологические механизмы компрессии СМК при рострокаудальном подвывихе, возможность мультифокальной компрессии СМК. Отражена роль пульпозного ядра как грыжевого материала в инициации воспалительного процесса, роль иммунных механизмов при дискогенных поясничных радикулопатиях. Отражена центральная роль цитокинов в развитии воспаления и болевого синдрома при дискогенных поясничных радикулопатиях. Показана важность изучения патофизиологических процессов с целью совершенствования методов диагностики и лечения дискогенных поясничных радикулопатий.</p></abstract><trans-abstract xml:lang="en"><p>The article provides an overview of current data on the pathophysiological mechanisms underlying the occurrence of lumbar disc radiculopathies. Microcirculation changes, axonal transport disorders at compression of the spinal root by a hernia of the intervertebral disc are described, the dynamics of a possible regression of intervertebral disc hernias is reflected. We have described the pathophysiological mechanisms of compression of the spinal root at rostrocaudal subluxation and the possibility of multifocal compression of the spinal root. We have reflected the role of the nucleus pulposus as a hernial material in the initiation of the inflammatory process, the role of immune mechanisms in lumbar disc radiculopathies. The crucial role of cytokines in the development of inflammation and pain syndrome in lumbar disc radiculopathies has been observed.
