Поражение костей скелета, часто сопровождаемое выраженным болевым синдромом за счет патологических переломов и/или компрессии нервных корешков или спинного мозга, является одной из причин нарушения качества жизни и снижения общей выживаемости у данной группы пациентов. Появление очагов остеодеструкции происходит в результате повышенной активности остеокластов и сниженной функции остеобластов. Однако влияние различных маркеров и цитокинов на процессы остеолиза при множественной миеломе ещё недостаточно изучено, в особенности на ранних этапах развития. Поэтому в настоящее время является актуальным изучение маркеров поражения костной ткани и факторов риска, которые позволят прогнозировать появление тяжелых поражений костей. Для определения потенциальных маркеров прогноза развития остеодеструктивных поражений были сопоставлены значения лабораторных показателей и иммуногистохимических (ИГХ) маркеров в группах пациентов с деструкциями костной ткани и без деструкций на момент диагностирования плазмоклеточных заболеваний. В результате установлено, что определение соотношения маркеров остеокальцин и β-CrossLaps, совместно с другими показателями (ИГХ CD 138+ &gt;10%, соотношение легких цепей k/λ &lt;0,1 и &gt;10) позволяет выделить группу пациентов с повышенным риском развития остеодеструктивного синдрома.</p></abstract><trans-abstract xml:lang="en"><p>Osteodestructive syndrome is one of the prominent signs of multiple myeloma. Damage to the bones of the skeleton, often accompanied by severe pain due to pathological fractures and/or compression of the nerve roots or spinal cord, is one of the causes of impaired quality of life and reduced overall survival in this group of patients. The appearance of foci occurs as a result of increased activity of osteoclasts and reduced function of osteoblasts. However, the effect of various markers and cytokines on the processes of osteolysis in multiple myeloma, and especially in the early stages of development, has not yet been sufficiently studied. Therefore, it is currently relevant to study markers of bone tissue damage and risk factors that can predict future severe bone damage.To determine potential markers of prognosis the development of osteodestructive lesions the importance of laboratory indicators and immunohistochemical (IHH) markers in groups of patients with bone lesions and without bone lesions at the time of the diagnosis of plasma cell diseases were compared.According to the results of the study, it was found that the ratio of β-CrossLaps/osteocalcin combined with other indicators (IHC CD 138+ &gt;10%, the ratio of light chains k/λ &lt;0.1 and &gt;10) makes it possible to identify a group of patients with an increased risk of developing osteodestructive syndrome for more frequent monitoring and can be used to diagnose osteodestructive syndrome at the stage of MGUS.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>моноклональная гаммапатия неуточненного значения</kwd><kwd>множественная миелома</kwd><kwd>гистохимические маркеры</kwd><kwd>биохимические маркеры костного метаболизма</kwd></kwd-group><kwd-group xml:lang="en"><kwd>monoclonal gammopathy of undetermined significance</kwd><kwd>multiple myeloma</kwd><kwd>histochemical markers</kwd><kwd>biochemical markers of bone metabolism</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">International Myeloma Working Group updated criteria for the diagnosis of multiple myeloma / S.V.
